Patient records

Every patient has one file. It holds the details the front desk enters, the history the doctor records, the forms the patient signs and the documents the clinic files.

What it does

  • A patient file with demographics, contact details, national ID, address, insurance details, emergency contacts, medical history, conditions and allergies.
  • An allergy alert banner on the patient record, built from the allergies recorded in the file.
  • Import of patient lists from Excel or CSV files, and export of the patient list to Excel.
  • Consent forms signed in the clinic, in the patient portal, or from a link the clinic sends. The link opens only after the patient confirms their date of birth, and the signed form is kept as a PDF.
  • Intake forms sent to the patient as a link, with the answers saved on the record.
  • Documents filed by category on the patient record.
  • Notes on the patient and on related records such as visits and appointments.
  • A data export per patient for access requests. Deletion requests get a 30-day grace period, after which the patient's data is anonymized.
  • Global search across patients and visits, opened with Ctrl+K.

How it fits

Visits, prescriptions and lab orders hang off the patient file, so the clinical record and the billing history are one click away. Patients see their own part of it in the patient portal.
